INITIAL CONSULTATION - Before starting treatment, a licensed massage therapist will meet with the client for an initial consultation. This will include going over the clients health history, discussing need for treatment, treatment options, and pricing. There are forms that will need to be filled out before beginning therapy. These forms may include health history, consent forms, HIPAA and information release, contact/emergency contact information, policies and procedures, and pricing and treatment agreement.
The initial consultation is FREE.
TREATMENT PLAN - Treatment may begin as soon as initial consultation is completed and all forms are signed and returned, and payment arrangements have been made. All appointment dates and times will be scheduled in advance. Payment arrangements and cancellation policy will be discussed at initial consultation.
FOLLOW UP - After recommended treatment is completed, there will be a follow up appointment to assess progress and evaluate need for continuing treatment.
